A formal ethics investigation has been launched into a doctor’s conduct following the death of a patient, Phanupong Wongwai, who allegedly exhibited symptoms of a heart attack but was reportedly diagnosed with anxiety. The Medical Council of Thailand (MCT) has accepted a complaint and initiated a comprehensive review of the case, aiming to determine if professional and ethical standards were met.
Patient’s Symptoms and Subsequent Death
The tragic sequence of events unfolded after Phanupong Wongwai’s wife, known on social media as Khru Sai, came forward with her account. She stated that her husband had complained of severe chest pain, pain that radiated to his arm, and excessive sweating. Despite these critical symptoms, she alleges that the attending doctor failed to perform an electrocardiogram (ECG) or conduct a thorough examination. Instead, the doctor reportedly attributed Phanupong’s condition to anxiety and discharged him.
Tragically, Phanupong collapsed shortly after being discharged. A subsequent autopsy revealed the underlying cause of his death: two blocked coronary arteries, a clear indicator of heart disease. This finding starkly contrasts with the initial diagnosis of anxiety.
Medical Council’s Investigation Process
ACM Ittaporn Kanacharoen, the secretary-general of the MCT, confirmed that the council has officially accepted the complaint. A dedicated team, including legal experts and the assistant secretary-general for ethics, has been appointed to oversee the investigation. The council’s approach will be thorough and evidence-based, involving a meticulous review of several key components:
- Medical Records: All documented patient information and treatment history will be examined.
- Autopsy Findings: The official cause of death and any related medical details from the autopsy report will be scrutinized.
- Doctor’s Explanation: The physician involved will be asked to provide their account of the events and the rationale behind their treatment decisions.
- Family’s Evidence: Information and statements provided by Phanupong’s family will be considered.
In addition to reviewing these documents, the MCT will coordinate with various institutions to gather a complete picture. This includes collaborating with San Camillo Hospital, where the incident occurred, as well as the Social Security Office and the Department of Health Service Support, and any other relevant governmental or healthcare bodies. The ultimate goal is to ascertain whether the medical care provided adhered to the established professional and ethical guidelines governing medical practice.
Hospital’s Response and Apology
San Camillo Hospital has responded promptly to the allegations and the subsequent investigation. Father Piyasak Wongwai, the hospital director, personally attended Phanupong’s funeral in Bangkok to offer a sincere apology to the grieving family. He acknowledged the hospital’s responsibility and pledged full cooperation with the MCT’s inquiry and any other official investigations.
The hospital has also taken internal measures. A separate probe committee has been established within the hospital to review the incident. Furthermore, the doctor implicated in the case, described as a part-time internal medicine specialist in his 70s with over a decade of service at the hospital, has been suspended pending the outcome of the investigations. Hospital officials have stated that they have been unable to contact the doctor since the incident occurred.
Father Piyasak emphasized the hospital’s commitment to transparency and cooperation with both the MCT and the Ministry of Public Health. He also expressed respect for any legal actions the family might choose to pursue. The hospital director noted the profound impact this tragedy has had on its staff and patient community, underscoring the need for the incident to serve as a critical learning experience. The hospital aims to implement improvements to prevent similar occurrences in the future, even as legal processes unfold.
Moving Forward
The investigation by the Medical Council of Thailand is expected to be impartial and strictly evidence-based. The findings will be crucial in determining accountability and reinforcing the importance of adhering to stringent medical ethics and professional standards. The outcome will likely influence patient care protocols and physician training, ensuring that critical symptoms are never overlooked and that all patients receive the highest standard of medical attention, especially when facing potentially life-threatening conditions.
